Knowing what to expect during your first visit can help put your mind at ease and reduce any worries you may have.

Here is some information for your initial visit to Community Care Physical Therapy.

When should I arrive?

Please arrive to your appointment 10 to 15 minutes prior to your scheduled appointment time to fill out any necessary paperwork. For your convenience, we have added forms to our website that can be printed off and completed prior to even entering our waiting room. Please click here to access those forms.

What should I bring with me?

Please bring the following items to your first visit:

  • Your insurance card
  • A photo ID
  • The prescription from your doctor
  • It may also be helpful to bring a list of any medications you are on
  • As a part of the multispecialty medical group Community Care Physicians, any imaging reports done within Community Care Physicians can be accessed using our shared electronic health records system. If you have imaging reports from the tests done outside of Community Care Physicians, please bring those with you to your appointment if you have them available.

How should I dress?

The physical therapist will need to access the affected area. You should wear loose fitting clothing so you are able to expose the area the physical therapist will be evaluating and treating

What will happen during my first visit with the physical therapist?

On your first visit, your physical therapist will perform a thorough evaluation. This evaluation will involve measurements of range of motion, posture, strength and balance. Your physical therapist will discuss your symptoms, how they are limiting your function and then develop a plan specific to your needs.
